Requirements
- Active Registered Nurse (RN) license in the state of Wisconsin
- Experience in home health and/or hospice nursing preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills to provide compassionate patient and family-centered care
- Ability to work independently in patients’ homes and as part of a multidisciplinary care team
- Reliable transportation for home visits within the Beaver Dam area
Key Responsibilities
- Conduct comprehensive nursing assessments for home health and hospice patients
- Develop, implement, and update individualized care plans in collaboration with interdisciplinary team members
- Administer medications, treatments, and interventions in accordance with physician orders and established protocols
- Provide education and support to patients and families regarding disease management, symptom control, and end-of-life care
- Document patient care accurately and timely in compliance with regulatory standards
- Monitor patient progress and communicate changes in condition to appropriate healthcare providers
- Maintain patient confidentiality and uphold ethical nursing standards at all times
Qualifications
- Current, unrestricted RN license in Wisconsin
- Previous home health or hospice nursing experience strongly preferred
- Basic Life Support (BLS) certification required
- Strong organizational skills with the ability to manage multiple patients and prioritize care effectively
- Empathy, patience, and dedication to quality patient care
